Deep TMS received FDA clearance as an adjunctive treatment for obsessive-compulsive disorder in 2018 — making it one of the few non-medication OCD treatments with formal FDA clearance. For patients who have not found adequate relief from therapy or medication alone, TMS offers a meaningful additional path.

How TMS Targets OCD

OCD involves problematic activity in the cortico-striato-thalamo-cortical (CSTC) circuits — a loop in the brain that, when dysregulated, drives the repetitive thoughts and compulsive behaviors that define the condition. TMS targets cortical areas connected to these circuits, particularly the dorsolateral prefrontal cortex (DLPFC), working to reduce the hyperactivity that sustains the OCD cycle.

The 2018 FDA clearance was specifically for deep TMS as an adjunct to ongoing treatment. In practice, that means TMS typically works alongside therapy or medication rather than replacing it entirely — though each patient’s situation is different.

What the Research Shows

Controlled clinical trials comparing active TMS to sham treatment have consistently demonstrated statistically significant reductions in OCD symptom severity in patients who had not responded adequately to first-line treatments. TMS consistently reduces OCD symptom severity when properly targeted — especially when focused on the DLPFC and its connections to the deeper CSTC circuit. The 2018 FDA clearance was the direct result of these sham-controlled trials.

Ongoing research continues to refine protocols and patient selection criteria. As with any psychiatric treatment, individual results vary, but the clinical evidence for TMS in OCD is the most rigorous that exists for any non-medication, non-surgical OCD intervention.

What Treatment Looks Like

TMS for OCD follows a protocol similar to treatment for depression: 36 sessions over approximately six weeks, five days a week, with sessions running about 20 minutes each. The treatment is outpatient, drug-free, and there is no downtime after your session.
